Choose <menuchoice><guimenu>File</guimenu><guimenuitem>New Entry</guimenuitem></menuchoice>. In the <guilabel>New Entry</guilabel> window, choose a <guilabel>Name</guilabel>, <guilabel>Comment</guilabel>, <guilabel>Command</guilabel> and <guilabel>Icon</guilabel>.
